Email date: 05/06/2011 Description: DEADLINE 5pm Monday May 9, 2011 Mandatory GIC Re-Enrollment REMINDER: Those who have not returned forms to Human Resources by 5pm on Monday, May 9 will be defaulted to the Community Choice plan and will not have an opportunity to change plans until open enrollment in 2012.
Remember -- even if you want to stay in the same plan, you MUST submit a re-enrollment form.
